How much can the jib boom lift at each position?

4,000 lb retracted (81"), 3,000 lb at the midway extension (109.5"), and 2,000 lb fully extended (138").

Will it fit my forklift forks?

The fork pockets measure 3.5" × 7.5" inside, with heel pins, so your forks must fit within that opening. The boom is secured with a safety chain and locking pin.

Does OSHA require anything when I add an attachment like this?

OSHA's powered industrial truck standard (29 CFR 1910.178(a)(4)) states: "Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly." Under 1910.178(a)(5), when a truck has front-end attachments other than factory-installed ones, "the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered."
